How Do I Access The University Of Tennessee - Knoxville (UTK) College Of Nursing Holistic Questionnaire?

To access the UTK College of Nursing Holistic Questionnaire:

1. Log in to the UTK SRAR using your SRAR/SSAR email address and password.
2. Click on the Record Complete tab.
   Tip: If you do not see the Record Complete tab, your SRAR/SSAR is in Edit Mode. Please submit your SRAR to access the Record Complete page and the UTK College of Nursing Holistic Questionnaire.
   
3. On the Record Complete page, click on the "College of Nursing Holistic Questionnaire" link to access the nursing questionnaire.
